Wellframe for Behavioral Health and Beyond

For patients with anxiety, depression, or other mental health conditions, having personal support can make all the difference when navigating stress caused by the pandemic. Your BlueCross BlueShield patients can now receive additional support right in the palm of their hand with Wellframe.                         

Wellframe is a digital care management solution that connects patients directly to a nurse, care manager, or health coach. Together, they set personalized goals that can help patients achieve better health.

The cloud-based mobile platform transforms patient care by displaying real-time care plan progress and clinical alerts for case managers and patients. There is also a chat feature where patients can ask questions, request resources, and more. Plus, additional articles are available on a range of topics to help patients learn more about wellness.

In addition to behavioral health issues, Wellframe offers more than a dozen programs for various health conditions. These include:

Signing up for Wellframe is easy. Your patients can download the Wellframe app from the App Store or Google Play and create an account using their BlueCross BlueShield member ID card and code BCBSWNY. You can also refer your patients to our Care Management team by calling 1-877-878-8785, option 2.
