Grants Awarded to WNY Organizations in Support of Community Health Outcomes
To: All Providers
Last month, 12 Western New York organizations were awarded Blue Fund grants for 2019 to support health-based community initiatives. These initiatives focus on behavioral health, cardiovascular health, diabetes, health care workforce development, healthy children, and the opioid epidemic.
Some of the recipients include:
Gerard Place – The Gerard Workforce Development Collaborative program will recruit and train qualified candidates for jobs in health care careers such as:
- Certified nurse assistants
- Home health aides
- Licensed practical nurse (LPN)
Kids Escaping Drugs – This organization is driving the Face2Face Program Expansion to expand the scope and reach of vaping education to area middle and high school students, as well as parents. Education initiatives will include webinars and virtual programming focused on vaping and its impact. The program will also focus on developing curriculum for Face2Face Summer Camp.
Population Health Collaborative – This program supports Live Well WNY by promoting community engagement, facilitating conversations and driving research to address how lack of exercise, poor diet, and tobacco use lead to cancer, heart disease, type 2 diabetes, and lung disease. These diseases contribute to more than 50% of all deaths, known as the 3-4-50 model.
Council on Addiction Recovery Services, Inc. – The Recovery Coach Expansion Project will extend the reach of recovery coaches to more strongly address substance use disorder (SUD). This initiative supports the increase in peer recovery coaches to patients struggling with SUD and can provide transportation to appointments.
Directions in Independent Living, Inc. – Directions: A Mobile Behavioral Health Project will provide a behavioral health van offering supportive services to youth, young adults, and families living in isolated rural communities and underserved urban neighborhoods in Cattaraugus and Allegany counties. Services will include:
- Peer support
- Social and recreational activities
These programs offer your patients and their families resources to connect with their communities and overcome health barriers. Click here for the complete listing of this year’s BlueCross BlueShield Blue Fund grant recipients.
