CDE-A Program Helps Diabetic Patients Lower A1C

Since it started in August 2017, our Certified Diabetes Educator Ambassador (CDE-A) program has helped more than 100 diabetic patients lower their A1C. The program’s early success offers promise as the COVID-19 pandemic continues to create new challenges for people with chronic conditions. Even better, this program is offered to your BlueCross BlueShield patients at no cost to them.

A recent study showed that:

  • Patients who completed the program decreased their A1C level by an average of 1%
  • 66% of participants decreased their A1C, and 36% had an A1C less than or equal to 7%
  • Even those who did not see the program through to completion managed to decrease their A1C by an average of 0.4%

Additional resources offered through the CDE-A program are available to your BlueCross BlueShield patients with diabetes, so they can further manage their condition. Our staff of Certified Diabetes Care and Education Specialists (CDCES) received expert training from endocrinologists Paresh Dandona, MD, PhD, and Ajay Chaudhuri, MD, and work closely with your patients to help lower their A1C to improve quality of life.

Patients receive guidance in the form of:

  • Self-management tools
  • Personalized wellness checklists 
  • Recommended screening tests and treatment goals
  • Free health coaching over the phone
  • Educational materials

In addition, Dr. Dandona and Dr. Chaudhuri consult with CDCES staff weekly to review patients’ medication regimes. Individual recommendations are then relayed to patients’ primary care providers.

By participating in our CDE-A program, patients actively work toward the goals you lay out for them. If you have a patient with an A1C level above 7% who may benefit from additional support, consider referring them to the program by calling 1-877-878-8785, option 2.

    To ensure proper processing and avoid denials or rejections, ancillary claims, lab orders, specialty pharmacy prescriptions, and referrals for your BlueCross BlueShield patients must be completed in full by the supervising physician of record.

    New drug therapy guidelines have been developed with the guidance of our Pharmacy and Therapeutics Committee. These guidelines will be posted by August 1 and will be effective September 1. Five medications will require preauthorization.
