Care at Home Expanded for Medicare Advantage Patients

Contracts affected: Medicare Advantage

Beginning on July 7, more Medicare Advantage patients will be eligible to enroll in a new extension of the Care at Home program -- Care at HomeSM On Call -- which offers some urgent care in-home services. Care at Home On Call will be offered to newly enrolled Medicare Advantage members, and will include:

  • An initial in-home assessment
  • 24/7 provider availability by phone

Both services will be provided by Landmark Health at no cost. Landmark currently provides in-home care to patients with multiple chronic and serious conditions through the Care at Home program. Interactions with patients will be reported to attributed primary care providers.

    New drug therapy guidelines have been developed with the guidance of our Pharmacy and Therapeutics Committee. These guidelines will be posted by August 1 and will be effective September 1. Five medications will require preauthorization.
