Wellness visits and preventive screenings are designed to keep you healthy. It's also safer and easier to prevent health issues than to treat them. If a potential problem is identified early through a wellness visit or screening, it can often be treated before it develops into something more serious.
Completing your annual wellness visit will give you a better picture of your health, and you’ll receive a $20 Prepaid Card as part of our Member Rewards.* In addition to receiving a $20 Prepaid Card for completing your annual wellness visit, you can earn an additional $20 Prepaid Card by completing a Breast Cancer Screening and/or a Colon Cancer Screening.
Earn up to $60 in total for all three screenings — one $20 Prepaid Card per screening. You’ll receive your $20 Prepaid Cards within 8-10 weeks of each completed visit. We encourage you to talk with your primary care doctor about which screenings tests are right for you.

What is an annual wellness visit?
The purpose of an annual wellness visit is to create or update your personal wellness plan. It doesn't cost you anything** and is different from a routine physical because your doctor will:
- Review your medical history
- Assess your risk factors
- Evaluate your physical and mental health
- Create a personalized wellness plan with you to develop a strategy for managing your health and plan the preventive services and screenings you may need for the next 5-10 years.

**Annual Wellness Visits, Breast Cancer Screenings, and Colon Cancer Screenings are covered by any doctor in our network as part of your member benefits; however, if other services are performed by your doctor at your visit, you may have a higher copay.
Can I schedule an in-home wellness visit?
You may receive a letter in the mail letting you know you are eligible for an in-home assessment. You'll then receive a call to schedule the visit with one of our nurse practitioners, or physicians assistants. If you do not receive a letter, you can request an in-home assessment by calling the customer service number provided on the back of your member ID card.
Breast Cancer Screening
Early detection and awareness are the best defense against cancer. A mammogram is a low-dose X-ray of the breast tissue and is used to screen for breast cancer. Having regular mammograms (every two years for women age 50 to 74 with average risk) can lower the risk of dying from breast cancer. If you are a woman ages 40 to 49 years, talk with your doctor to see if a mammogram is right for you.

Colorectal Cancer Screenings
People 50 or older are statistically at a greater risk for colorectal cancer. That’s why health professionals recommend screening for colorectal cancer starting at age 50. People at a high risk for colorectal cancer may need to start screening at an earlier age.
For most people, a colonoscopy is only needed once every 10 years, although people at higher risk may need to be screened more often. There are several other screening options in addition to colonoscopy, such as stool sample testing (FIT or fecal occult blood tests, FIT-DNA), CT colonography, and flexible sigmoidoscopy.
If you’re 50 or older, please talk with your doctor about the benefits of colonoscopies and other colorectal cancer screenings. Together, you can decide which is best for you.
