What is the Cardiac Management Program?

Prevention of cardiac disease starts early, and, in many cases, before heart disease is diagnosed. Our program focuses on members with diabetes, high blood pressure (hypertension), and high cholesterol (hyperlipidemia) as well as those with prior cardiovascular events. By effectively managing these conditions, the serious and often life-threatening effects of heart disease can be prevented or managed. 


In addition, if you or a loved one underwent heart surgery such as a stent placement or open heart surgery, our team of specialized care managers will work with you and your health care team to promote healing and make the most of recovery. 


For those suffering from congestive heart failure, our program focuses on improving quality of life. By providing education regarding diet, weight management, and proper use of medications, unwanted trips to the hospital can be avoided. 


Who is eligible for the program?
Members with a diagnosis of heart disease, high blood pressure, or high cholesterol are eligible to join the program as well as those members who have suffered a cardiovascular event. Members are identified through:

  • In patient hospital claims
  • Out patient medical claims
  • Referral from a doctor
  • Self-enrollment either online or by phone
