Asthma Management Program

The Asthma Management Program encourages you to take charge of your asthma by using your health care team to develop personalized management tools. When patient self-care education starts early in the disease, it can help prevent life changing complications. 

All members with a diagnosis of asthma are eligible to join the program. Members can be identified for eligibility through:

  • Out-patient medical claims
  • In-patient hospital claims
  • Pharmacy claims
  • Members can be referred to the program by their health care providers
  • Members can enroll themselves in the program

  • Personalized educational material regarding asthma, including an Asthma Action Plan
  • Free, telephonic health coaching by our staff of registered nurses
  • Newsletter articles
  • Web links to nationally recognized leaders in asthma management
  • Web-based program information and self-management tools
  • Personalized medication profile and wellness checklist to promote effective use of "controller" medications
  • Comprehensive prescription formulary
  • On-line enrollment and "opt-out options"

Asthma is a disease that affects your lungs. It causes repeated episodes of wheezing, breathlessness, chest tightness, and nighttime or early morning coughing. Asthma can be controlled by taking medicine and avoiding the triggers that can cause an attack. Click the links below to learn more.
