When you have multiple medical needs, coordinating everything you need can be exhausting.
At BlueCross BlueShield of Western New York, we want to help you have less to worry about.

We have partnered with Landmark Health to create Care at Home - a no-cost, in-home service that complements your
doctor's treatment plan, and assists you and any family members or friends who may be involved in your care.

Care at Home professionals

You will continue to see your own primary care doctor, but will also have the support of a team of health care professionals that includes:

  • Doctors, nurse practitioners, and nurses
  • Physician assistants
  • Behavioral health providers
  • Pharmacists
  • Dietitians and social workers


Care at Home can help:

  • Schedule and prepare for appointments
  • Make sure you have your medications, and learn the best way to take them
  • Identify early signs of worsening health to avoid emergency room visits or hospital stays
  • Find additional resources and services to keep you healthy at home

For a decision about whether we will cover an out-of-network service, we encourage you or your provider to ask us for a pre-service organization determination before you receive the service. Please call our customer service number or see your Evidence of Coverage for more information, including the cost-sharing that applies to out-of-network services. 

Medicare evaluates plans based on a 5-star rating system. Star Ratings are calculated each year and may change from one year to the next.
